Service Overview
The Name Change Request Service is offered to all Niagara College faculty, staff, and students.
Employees: Prior to filling out this form, please review the name change request process outlined on the Employee Hub. ITS will not be able to process requests that have not followed this process.
Students: Prior to filling out this form, please review the name change request process outlined on the Enrolment Services website. ITS will not be able to process requests that have not followed this process.
**Please Note: Changing your name will typically result in a change to your username and email address. When the name change is complete, an email notification will be sent to your personal email account (if provided), or to the employee submitting the Name Change Request, containing your new email address and username. Your old mailbox will be renamed to your new mailbox, and your old email address will continue to forward mail to your new email address.
Don't forget to let all your contacts know your new email address as soon as you receive it.
