Transition Report

Purpose

The Transitions report is used to identify what needs to be done to hand over the deliverable to operations and support teams. Completing this document will help to facilitate a smooth transition after the project closes, and set expectations as to how issues will be handled and escalated once the project team disbands

Transition To Operations Report

Guidelines

  • Should be completed by the Project Manager and project team members and reviewed with the operations staff who will be taking ownership of the solution after the project has been implemented and closed.

Transition Tasks

  1. Set up and coordinate transition to operations planning meeting
  2. Establish transition milestones
  3. Determine roles and responsibilities
  4. Identify support process to be followed
  5. Identify major incident scenarios
  6. Obtain access/privileges
  7. Identify documentation required and created
  8. Develop training plans
    • Identify any cross training requirements
    • Identify training cources
    • Schedule classes
    • Identify necessary knowledge transfer activities
  9. Notify business area (s) of production support proecure
  10. Notify Help Desk
  11. Update Service Catalogue
